Registration & Insurance Verification
Our Client Care team will review your request and verify your insurance coverage.

Client Signs Consent & Agreement
You'll receive secure intake paperwork to review and sign electronically.

Your Appointment Will Be Scheduled & Confirmed
Once all steps are complete, your first session is confirmed and your care begins.
What you'll need: Your insurance card (front & back photo), primary insurance member ID, and the name and phone number of an emergency contact. This form takes approximately 5 minutes to complete.
