Step 1: Complete the Appointment Request Form
The process begins with a short online form.
This form gathers basic information and gives you space to briefly share what’s bringing you in. You don’t need to have everything figured out before completing it. Many people start with uncertainty or even difficulty putting their experience into words — that’s completely valid.
This first step is simply about opening the door. It allows the team to begin understanding how to support you without requiring a full explanation upfront.
